Cancellation Policy

ACKNOWLEDGMENT OF LIMITED SERVICES AND CONSENT TO DISCLOSURE OF INFORMATION The Mission First Legal Aid Office (“Mission First”) has agreed to provide me with a one- time meeting regarding my legal matter. At this time, there is no attorney-client relationship between Mission First and me, and Mission First has not agreed to represent me. Mission First will only establish an attorney-client relationship and provide legal representation after Mission First and I sign a separate engagement letter. I am not currently represented by an attorney regarding this matter. The purpose of this meeting is to review and discuss the legal matter for which I have requested assistance. To the extent possible and appropriate, Mission First will provide information and advice regarding this matter. If Mission First determines that it can provide additional assistance, an engagement letter will be prepared. Unless and until an engagement letter is signed, if I desire representation from an attorney, I am responsible for finding and retaining an attorney. I understand that Mission First relies heavily on volunteer attorneys and may need to discuss my situation with them. I consent to the disclosure by Mission First of any information and documentation regarding me and my legal matter to any person or entity deemed appropriate by Mission First. I authorize Mission First to use information about my matter for statistical, research, or academic purposes. I unconditionally release and/or waive claims, known or unknown, of any kind arising out of or related to the advice, referral, or service, or the lack of any advice, referral, or service, which I may now or in the future have against Mission First or any of its affiliates, employees, volunteers, or agents. By booking this appointment, I acknowledge that I have read, understood, and agreed to the terms and policies contained herein.
